Appointments are for medical concerns only.

For forms or paperwork, please submit through the “Form, Note or Paperwork Option"

Submit all forms (sick notes, insurance, disability, etc.) through this option.

We’ll let you know if an appointment is required to complete your paperwork.

Prescriptions require medical monitoring, so appointments are recommended.

For refills without a visit, have your pharmacy fax us. A $30 fee applies and refills may take up to 5 business days after payment.

For other requests such as record transfers, general inquiries, or administrative help.
