Prescriptions require medical monitoring, so appointments are recommended. To request a refill without an appointment, ask your pharmacy to fax us. A $30 fee will be emailed to you. Once paid, the doctor will issue a refill if appropriate. Please allow up to 5 business days after payment.

For other requests such as record transfers, general inquiries, or administrative help.
